Our goal is to provide home based services, supportive living services and community day services for individuals with Intellectual and Developmental Disabilities so that they can live in their own home as well as an individual's family home Community Care Team (CCT) has immediate openings for enthusiastic and dependable In-home-Support and Individualized-Day-support staff for individuals with Developmental / Intellectual Disability and Autism.
Support staff is responsible for providing one-on-one support/safety due to personal support needs.
Support the individual while they participate in family and community events to include social events, leisure time activities, family interactions/activities and meals.
Guidance in personal care needs.
Support Staff role:
Providing one-on-one support/safety due to personal support needs.
Support individual while they participate in family and community events to include social events, leisure time activities, family interactions/activities and meals.
Serve as friend, advisor and companion.
Guidance in personal care needs Build the relationship by planning and participating in activities together.
Complete daily progress notes and program specific data Build self-esteem and motivation.
Strive for mutual respect.
In home support staff will perform job duties as assigned by their supervisor.
Maintain all required training standards and health requirements within specified time frames.
Must provide supports of day-to-day activities such medical appointments.
Must keep an open line of communication with I.
H.
S Coordinator and notify any and all imperative information surrounding the person supported.
May provide hands-on support to successfully complete activities of daily living tasks to include:
eating, bathing, dressing, toileting, oral hygiene and hair care.
Required to perform behavioral interventions and techniques as needed.

Part-time:
Monday - Friday shift 9am - 3pm Monday -Friday shift 3pm - 10pm Weekends:
10am to 5pm (flexible hours) Job Type:
